Health and Safety Inspections
What is a Health and Safety Inspection?
Most child care providers offer safe and healthy environments. Health and safety inspections and investigations are a normal part of child care in Idaho. Inspections are not intended to "catch" anyone doing something wrong. Inspections help ensure the safety of children and staff, support staff in following requirements by finding concerns, and promote continuous improvement.
Idaho began publishing the results of health and safety inspections October 2017. Inspections are published as they are conducted and maintained on this site for at least three years.

Incident Reports
What is a Substantiated Incident?
Occasionally, families and community members may report that a child care facility is not meeting requirements for safe and healthy child care. All child care incidents reported in Idaho are investigated. After an investigation, if it is determined by the relevant authorities that an incident occurred, it is considered “substantiated.” It is important to know that only substantiated incidents and allegations are published on this website. Incidents that are still under investigation will not be published until the investigation is concluded.
Idaho began publishing substantiated incidents in October 2017. Incidents are published as each investigation is concluded and maintained on this site for at least three years.
Serious Injury
A serious injury was reported by the child care facility. During the investigation, it was confirmed a child slipped and fell on a slide at the park and hit his tooth.
Resolution
The child was comforted and a compress was put on the mouth to stop the bleeding. Parents were called and picked the child up. Provider implemented new schedule changes to stagger class recess times, and required all staff to take Active Supervision TTA Training to better prepare teachers.
Serious Injury
A serious injury was reported by the child care facility. During the investigation, it was confirmed a child tripped in the classroom and hit their head on the bottom shelf of a cubby.
Resolution
The provider implemented new safety measures and required all staff to participate in Active Supervision training.
Serious Injury
There were concerns a child tripped on uneven pavement while playing outside at recess. During the investigation it was found that the child was taken to the emergency room and needed 8-9 stitches for a gash on their forehead. The health department verified that ratios and supervision were in compliance at the time of investigation and during the time of the incident.
Resolution
Provider advised to ask the University to replace the uneven sidewalk to prevent future injuries. SInce ratios and supervision were in compliance during the incident, there is no further resolution needed.
Supervision Concerns
There were concerns regarding staff not supervising children properly in one of the rooms. During an investigation, it was determined that a certified teacher was not in a classroom at all times and an aid was left alone with children.
Resolution
The director is putting a certified staff member in the room on a permanent basis.
Sand in Diaper
A child had a rash going up their back and appeared to be from scratching at their diaper. When the child's diaper was changed by the parent, they noticed it was full of sand. Facility states that the child's diaper had been changed since being outside and was logged on their documentation log for changing of diapers. Provider believes that the sand in the diaper fell from their shirt after his diaper was changed.
Resolution
Provider believes that the sand came from the shirt and fell after the diaper was changed. The Provider will check for this in the future after a child has played outside.
